Stevenson Memorial Hospital launches Patient Connect online portal

Patients at Stevenson Memorial Hospital (SMH) will have access to their health care information at their fingertips 24 hours a day, seven days a week with the launch of a new portal called Patient Connect.

Patient Connect is an online tool that gives patients and families the ability to view:

  • upcoming appointments and procedures
  • laboratory results
  • diagnostic imaging reports
  • notes from healthcare providers
  • visit history information
     

The launch of Patient Connect takes SMH to Phase 2 of the implementation of the hospital’s new Health Information System (HIS) Meditech Expanse, in partnership with Southlake Regional Health Centre and Markham-Stouffville Hospital. Phase 1 of the implementation took place on December 1, 2018 with a new electronic medical records system. Physicians and medical professionals have seamless access to patient information across all three sites, improving patient safety and supporting faster diagnosis and treatment.

Now patients will have access to their own information in Patient Connect, which includes health care information from all three hospitals in the partnership.

“We’re thrilled to be able to offer this opportunity for patients,” says William Bye, VP and Chief Financial & Information Officer, SMH. “This means they will receive test results sooner and have access to them anytime, whether the test was done at Stevenson, Southlake or Markham-Stouffville. This will help all patients, but especially those with complex care needs,” he adds.

To enroll in Patient Connect at SMH:

  • Obtain a brochure and form at registration (main entrance).
  • Complete the form and place it in the drop box marked ‘Patient Connect’ in the lobby.
  • The Health Information Department will process the form.
  • Watch for an email with the link to complete your login/enrolment.
