Nominate an SMH Nurse for the Award of Excellence in Nursing
We’re asking residents of New Tecumseth and surrounding communities to nominate someone in the Nursing Profession – an RN, RPN or Nurse Practitioner – for the SMH Award of Excellence in Nursing.
Submit your nomination to us, including the following information:
- First and last name of the nurse
- Where the care or association with the nurse took place (specify hospital ward, health care location, department or organization)
- Approximate dates of care provided, or date of association with the nurse
- Your name and daytime telephone number
- Your relationship to the nurse (i.e. patient, family member of patient, friend of patient, former student, or colleague).
Then, in 250 (minimum) to 500 words, provide a personal account explaining why this nurse deserves to be recognized with the Award of Excellence in Nursing. This may include a brief description of his or her qualities, care of patient, specific examples of the nurse’s efforts and dedication, and the difference he or she has made to your life or the lives of others. (NOTE: Your story must have taken place between April 1, 2020 and March 31, 2021. Nominee must be registered with the College of Nurses of Ontario. Nurses may not be nominated by a member of their family.)

Deadline for Nominations: Friday, June 18, 2021
A Panel of Judges will select a recipient, who will be announced and presented with the award during SMH’s Virtual AGM on Tuesday, June 22, 2021.
